I've went over many pages of this thread and have yet to see mention of the problem I've had: hub isn't square. Put on a freshly turned rotor, bolt it to the hub without a wheel, use a rotor runout gauge and clock the rotor as perfectly as you can. If you can't get it within 2 thou, try again with a shim.
